What Is Shock Loss?
Shock loss refers to temporary hair shedding occurring in the weeks following a hair transplant. It can affect transplanted hair and surrounding native hair. Both are normal, expected, and temporary.
Why Do Transplanted Hairs Shed?
When follicles are transplanted, they experience significant trauma and respond by entering a resting (telogen) phase — the hair shaft is shed while the follicle goes dormant. The follicle itself remains alive beneath the scalp surface. After 2–3 dormant months, it re-enters active growth (anagen) and begins producing a new hair shaft.
Why Does Native Hair Also Shed?
Thousands of tiny incisions created during surgery disrupt the local tissue environment, pushing surrounding native follicles into telogen. This is temporary — affected follicles are not permanently damaged in the vast majority of cases.
Timeline
Transplanted hairs typically begin shedding weeks 2–4. Native hair shock loss occurs on a similar timeline. The dormant phase lasts approximately 2–3 months. New growth begins from month 3–4, with meaningful coverage visible by month 6.
Who Is Most at Risk?
Shock loss in native hair is more pronounced in patients with already fine or miniaturised native hair, those undergoing large sessions, and those with weakened follicles from underlying hair loss conditions.
How to Minimise Shock Loss
Minoxidil from week 4 onwards is commonly recommended to support the transition back to anagen. Correct post-operative care — gentle washing, avoiding trauma — minimises disruption to the healing environment.
