DHI — Direct Hair Implantation — is one of the two primary hair transplant techniques used at leading clinics worldwide. It offers distinct advantages over standard FUE for certain patients, and understanding how it works helps you make an informed choice with your surgeon.
How DHI Works
In standard FUE, the procedure involves three separate steps: extraction of follicular units, creation of channels in the recipient area, and implantation of grafts into those channels.
DHI combines the final two steps into one. Using a specialised tool called the Choi Implanter Pen, the surgeon loads a follicular unit directly into the pen and implants it into the scalp in a single motion — creating the channel and inserting the graft simultaneously.
The result: less time between extraction and implantation, less handling of each graft, and greater precision over the depth and angle of each implanted follicle.
The Choi Implanter Pen
The Choi pen is a hollow needle attached to a plunger mechanism. Available in various diameters (0.6–1.5mm) to suit different graft sizes, it allows the surgeon to:
- Control implantation depth precisely
- Set the exact angle of each graft (critical for natural-looking results)
- Implant without disturbing surrounding hair
- Work with the recipient area unshaved in most cases
Multiple pens are used throughout the procedure — while one technician loads grafts into a pen, the surgeon implants with another, minimising graft exposure time.

Who Is DHI Best Suited For?
DHI is particularly well-suited to:
Patients at Norwood stages I–IV who need a moderate number of grafts (up to 3,500). For more advanced cases, the limited graft count per session may make FUE a better choice.
Patients who don't want to shave the recipient area. Since DHI doesn't require pre-made channels, the existing hair in the recipient area doesn't need to be removed. This is a significant advantage for patients who want to maintain their current appearance during recovery.
Patients adding density to existing hair. DHI's precision makes it ideal for increasing density in areas that are thinning but not yet fully bald — placing grafts between existing hairs without damaging them.
Patients prioritising graft survival. The reduced handling time and combined extraction-implantation process means less graft exposure, which can translate to marginally higher survival rates.
The DHI Procedure: Step by Step
1. Consultation and hairline design: Your surgeon maps the recipient area and designs the implantation zones.
2. Donor area preparation: The donor area (back and sides of scalp) is trimmed and local anaesthetic applied.
3. Extraction: Follicular units are extracted with a micro-punch, as in standard FUE.
4. Graft loading: Technicians load extracted grafts into Choi implanter pens.
5. Implantation: The surgeon implants grafts directly into the scalp using the pens, controlling angle, depth and direction for each follicle.
6. Post-operative care: Donor area is bandaged, aftercare kit provided.
Total procedure time: 6–8 hours.
Recovery After DHI
Recovery after DHI is broadly similar to FUE, but slightly faster due to less tissue disruption in the recipient area.
- Days 1–7: Mild redness and small scabs at implantation points
- Week 2–3: Scabs resolve; shock loss may begin
- Month 3–4: New growth starts
- Month 12: Full results visible
Because the recipient area isn't shaved, the recovery is often less visually obvious — an advantage for patients who cannot take extended time away from work or social life.
