One of the first questions patients ask is: how many grafts will I need? The answer depends on your hair loss stage, your desired density, and the quality of your donor area. This guide gives you a clear framework to estimate your graft requirement before your consultation.
What Is a Graft?
A graft is a naturally occurring group of hair follicles harvested from your donor area (typically the back and sides of the scalp). Each graft contains between 1 and 4 hairs. When surgeons quote a graft number, they mean the number of follicular units transplanted — not individual hairs.
This is why two patients with the same graft count can end up with different hair coverage: someone with mostly 3-hair grafts will achieve denser results than someone with mostly 1-hair grafts.
Graft Estimates by Norwood Stage
The Norwood scale is the standard classification for male pattern hair loss, ranging from Stage 1 (no loss) to Stage 7 (extensive loss). Here are typical graft requirements:

These are estimates. Your actual requirement depends on your personal hair characteristics.
Factors That Affect Your Graft Count
1. Hair calibre (thickness) Thicker hair shafts provide more coverage per graft. Patients with naturally thick hair often need fewer grafts to achieve the same visual density as someone with fine hair.
2. Hair colour contrast Dark hair on light skin creates the highest contrast — and can make thinning more visible. Patients with lower contrast (blonde or grey hair on fair skin) often achieve good coverage with fewer grafts.
3. Curl and wave Curly or wavy hair covers more surface area per graft than straight hair. African or mixed-heritage patients often achieve excellent density with a lower graft count.
4. Donor area density The back and sides of your scalp contain your permanent hair. If your donor area is dense, you have more grafts available. If it is thin, your surgeon needs to plan carefully to avoid over-harvesting.
5. Target area size A patient restoring only their frontal hairline needs far fewer grafts than one trying to cover the entire top of the scalp including crown.
How Many Grafts Can Be Transplanted in One Session?

What Happens If You Run Out of Donor Hair?
Donor supply is finite. Once follicles are harvested from the scalp donor area, they cannot be replaced. This makes careful planning essential — especially for younger patients whose hair loss may continue to progress.
Options when scalp donor supply is limited:
- Body hair transplant (BHT): Hair from the beard, chest or legs can supplement scalp grafts. Beard hair is the most commonly used and provides excellent results.
- Lower density approach: Focusing grafts on the frontal third of the scalp creates the most visible improvement with the fewest grafts.
- Staged procedures: Spreading the transplant across two sessions preserves more control over long-term planning.
