The donor area — typically the occipital and temporal regions of the scalp — is the source of permanent hair grafts. Unlike hair in the top of the scalp (which is sensitive to DHT), donor zone hair is genetically resistant to DHT and, once transplanted, maintains this resistance in the new location. This is what makes hair transplant a permanent solution.
What Is the "Safe Zone"?
The safe zone is the area of the scalp where donor hair is most reliably permanent. It forms a horseshoe shape at the back and sides of the head. The exact boundaries of the safe zone vary by individual — and in patients with advanced hair loss, the safe zone may be narrower than in patients with stable Norwood 3 loss.
Harvesting outside the safe zone carries the risk that some grafts will eventually be affected by DHT — undermining the permanence of the transplant.
How Donor Capacity Is Assessed
Density: Hairs per cm² in the donor area. Average density is approximately 70–90 FUs/cm². Higher density means more available grafts.
Laxity (for FUT): How much the scalp moves — determines strip width in FUT procedures.
Area: The physical size of the safe zone determines the total harvestable area.
Average grafts per FUE extraction point: Some patients have predominantly multi-hair grafts (2–3 hairs per follicle), dramatically increasing yield per extraction.
Estimated total lifetime capacity: For FUE, typical safe lifetime extraction is 4,000–7,000 grafts depending on density, hair calibre, and scalp size.
Why Donor Preservation Matters
Donor supply is finite. Every graft extracted permanently reduces the available supply. A young patient who uses 4,000 grafts at age 22 for Norwood 3 coverage may have inadequate remaining supply to address progression to Norwood 5–6 by age 40.
Good clinics plan procedures with long-term trajectory in mind — preserving sufficient supply for likely future needs.
Poor clinics maximise graft count per session for marketing purposes ("5,000 grafts in one session!") without regard for lifetime planning.
What Happens if Donor Is Depleted?
Options for patients with depleted scalp donor:
