Minoxidil is a topical (and now oral) medication that promotes hair growth and is widely used both as a standalone hair loss treatment and as a post-operative support medication after hair transplant.
Why Minoxidil Is Used After Hair Transplant
Reducing native hair shock loss: Minoxidil keeps hair follicles in the anagen (growth) phase — potentially reducing the severity and duration of post-transplant shock loss in native hairs.
Supporting transplanted graft establishment: Some evidence suggests minoxidil may support blood flow and the establishment of new blood supply networks around transplanted follicles.
Protecting ongoing native hair loss: The most important long-term reason — the native hair surrounding transplanted areas continues to be affected by DHT. Minoxidil (combined with finasteride where appropriate) slows ongoing loss.
When to Start After Hair Transplant
Most protocols recommend starting minoxidil 2–4 weeks post-procedure — once initial healing of the recipient area is complete. Starting too early risks disturbing the recently implanted grafts through foam/liquid application.
How Long to Use It
Minoxidil is not a cure — it must be used continuously to maintain its effect. Stopping minoxidil typically results in reversal of any benefit within 3–6 months. Patients should understand this is a long-term commitment.
Topical vs Oral Minoxidil
Topical: Applied to the scalp twice daily. Most widely used form. Side effects: scalp irritation in some patients.
Oral (low-dose): Increasingly prescribed at 0.625–2.5mg daily. More consistent absorption. Side effects: mild fluid retention, initial increased shedding (temporary), hypertrichosis (unwanted body hair in some patients).
