Scalp micropigmentation (SMP) and hair transplant are often compared — but they are fundamentally different interventions with different outcomes, different use cases, and different maintenance requirements.
What Is Scalp Micropigmentation?
SMP is a cosmetic tattooing procedure where pigment is deposited into the scalp skin using a micro-needle, replicating the appearance of hair follicles. The result is the appearance of a closely shaved head with defined hair stubble — or the illusion of density in thinning areas.
What SMP Can and Cannot Do
Can do:
- Create the illusion of a shaved-head look with defined hairline
- Add the appearance of density to thinning (but not bald) areas
- Camouflage donor area scarring from FUT strip procedures
- Create a defined hairline without surgical recovery
Cannot do:
- Produce actual hair growth
- Work in completely bald areas where hair cannot be parted or styled over
- Provide the natural movement, styling versatility, or feel of actual hair
- Work for patients who want to wear their hair longer than a very short shave
When SMP Is Appropriate
SMP suits patients who: prefer the shaved-head aesthetic and want a more defined hairline; cannot or do not want surgery; have donor area FUT scarring requiring camouflage; or want to complement a hair transplant by adding density illusion to areas between transplanted hairs.
When Hair Transplant Is More Appropriate
Hair transplant suits patients who: want actual hair growth; want styling versatility (longer hairstyles); have specific bald areas to restore; have sufficient donor supply for their coverage goals.
Combining SMP and Hair Transplant
Some patients benefit from both. Hair transplant restores actual hair in priority areas; SMP adds apparent density between transplanted hairs or in areas not covered by the transplant. The combination can maximise the aesthetic result for patients with advanced hair loss.
Permanence and Maintenance
Hair transplant results are permanent (the transplanted hair). SMP fades over time — typically requiring touch-up sessions every 3–5 years as the pigment lightens.
