One of the most common concerns patients raise before booking a hair transplant is scarring. The good news: modern FUE and DHI techniques leave minimal, largely invisible marks — but understanding what to expect helps you prepare, protect your results, and avoid complications.
Why Older Techniques Left Visible Scars
The FUT (Follicular Unit Transplantation) strip method, which dominated hair restoration before 2010, removed a horizontal strip of scalp from the donor area. This left a linear scar that became visible whenever the hair was cut short. Many patients who had FUT procedures decades ago still carry that scar.
FUE and DHI changed this entirely. Instead of removing a strip, individual follicular units are extracted one by one using a precision punch tool — leaving only tiny circular micro-wounds, typically 0.7–0.9 mm in diameter.
What Scarring Looks Like With FUE and DHI
With modern Sapphire FUE or DHI, scarring is characterised by:
Donor area: After extraction, each punch site heals as a small white dot. When hair grows back — which typically begins within 4–6 weeks — these dots become invisible to the naked eye. They are only visible if the scalp is shaved completely.
Recipient area: Tiny incisions are made in the thinning area to receive the transplanted grafts. These heal within 7–10 days and leave no visible marks once the transplanted hair grows in.
Most patients can keep their hair at a grade 2–3 buzz cut (5–8 mm) without any visible evidence of a transplant having taken place.
Week-by-Week Healing of the Donor Area
Days 1–3: The donor area appears red and slightly swollen. Tiny scabs form around each extraction point. This is normal.
Days 4–10: Scabbing begins to resolve. You may notice mild itching — do not scratch. Continue washing gently with the clinic-provided shampoo.
Weeks 2–4: Redness fades. Transplanted hair in the recipient area begins to shed (shock loss) — this is expected and temporary.
Weeks 6–12: Donor area looks normal. The extraction dots have largely healed to flat, pale marks. Hair starts to regrow in donor area.
Months 3–6: Donor area fully recovered. Early transplanted hair growth begins.
Months 9–18: Final result visible. Donor scars, if any, are indistinguishable from surrounding scalp.
Factors That Affect Scarring
Punch size: Smaller punch diameters (0.7–0.8 mm) leave smaller wounds that heal more completely. At Estetİstanbul we use 0.8 mm or smaller punches.
Surgeon skill: Experienced surgeons extract grafts at the correct angle and depth, minimising trauma to surrounding tissue. Poor technique causes enlarged wounds and irregular scarring.
Skin type: Patients with olive or darker skin tones may experience slightly more noticeable hyperpigmentation during healing, though this typically fades. Keloid-prone individuals should discuss this with their surgeon before proceeding.
Post-operative care: Picking scabs, sun exposure on the healing scalp, or early physical activity increases scarring risk.
How to Minimise Scarring After Your Procedure
Do not pick or scratch scabs. Allow them to fall naturally during washing.
Protect the scalp from sun. UV exposure on healing skin can cause hyperpigmentation. Avoid direct sun on the scalp for 4–6 weeks and use SPF 50 if unavoidable.
Follow the washing protocol. The clinic will provide a specific washing regimen. Adhering to it prevents infection and promotes clean healing.
Avoid smoking. Nicotine restricts blood flow to healing tissue. Stopping smoking for 2 weeks pre- and post-procedure significantly improves healing quality.
Take prescribed medications. Anti-inflammatory and antibiotic courses are prescribed to prevent infection and reduce swelling. Complete the full course.
Can Existing Scars Be Treated?
Patients who had FUT procedures in the past and have a visible linear scar have options. FUE grafts can be transplanted directly into the scar tissue to camouflage it — a technique called scar camouflage. Results depend on the scar's age, width, and vascularity.
