Traction alopecia is one of the more preventable causes of permanent hair loss. Caused by chronic mechanical tension on hair follicles, it is most commonly seen in women who wear their hair in tight braids, extensions, weaves, buns, or ponytails — but can affect anyone whose hairstyle places sustained tension on the hairline or scalp.
What Causes Traction Alopecia?
Prolonged mechanical tension on hair follicles disrupts the normal hair cycle, causing hairs to enter the resting (telogen) phase prematurely. Over time, the sustained tension causes the follicle to miniaturise — producing progressively finer hair until it eventually ceases production entirely. If the tension is maintained for long enough, follicle damage becomes permanent and scarring occurs.
The hairline, temples, and the nape of the neck are the areas most commonly affected, as these regions experience the greatest tension from typical tight hairstyles.
Who Is at Risk?
Traction alopecia is disproportionately common in:
- Women of African and Afro-Caribbean descent who wear braids, cornrows, or extensions
- Ballet dancers, athletes, and others who wear hair tightly pulled back for long periods
- Men and women who wear very tight hair ties or ponytails habitually
- Users of hair extensions, weaves, or wigs attached with tension
Recognising Traction Alopecia
Early signs include:
- Tenderness or soreness at the hairline after wearing a tight style
- Small pimples or follicle inflammation (folliculitis) at the hairline
- Progressive recession of the hairline, particularly at the temples and frontal hairline
- Shorter, finer regrowth along the hairline
In early stages, the loss is reversible if the causative tension is removed. In advanced stages, loss becomes permanent.
Treatment
First step: eliminate the cause. This is non-negotiable. Without removing the source of tension, no treatment will produce lasting results. Switching to looser hairstyles, avoiding extensions, and reducing tension on the hairline is essential.
Early stages: If follicle damage is not permanent, regrowth may occur spontaneously once tension is removed. Minoxidil can support this process.
PRP therapy: Platelet-rich plasma injections into the affected hairline may stimulate follicle recovery in cases where damage is not fully permanent.
Advanced stages with permanent loss: Where follicles are permanently damaged or lost, hair transplantation can restore the hairline. The condition must be fully stable and the causative hairstyling practices permanently changed before transplant is undertaken.
