Traction alopecia is hair loss caused by repeated or prolonged mechanical tension on the hair follicle. It is one of the most common causes of hair loss in women — particularly those who frequently wear tight braids, weaves, extensions, ponytails, or buns.
How Traction Alopecia Develops
Repeated tension on the hair follicle disrupts the normal hair cycle and causes inflammation around the follicle. Initially, this produces reversible hair loss — the follicle is stressed but not permanently damaged. With continued tension over months or years, the inflammation becomes chronic and can permanently damage or destroy the follicle — at which point the hair loss becomes irreversible.
The characteristic pattern is hair loss at the hairline (frontal and temporal areas), where hairstyle tension is greatest.
Early Signs
- Progressive thinning at the hairline
- Small, fine hairs or "baby hairs" breaking around the hairline
- Scalp tenderness or pain when wearing hair up
- Small follicular pustules or bumps at the hairline
Treatment
Stop the causative hairstyle immediately: The single most important intervention. If follicles are not yet permanently damaged, removing the source of tension allows recovery over months.
Anti-inflammatory treatment: Topical corticosteroids or intralesional steroid injections reduce inflammation and can support follicle recovery.
Minoxidil: Can support regrowth in areas where follicles are dormant but not destroyed.
Trichoscopy assessment: A dermatoscopic examination determines which areas still have follicular openings (potentially recoverable) versus areas with follicular dropout (permanent loss).
When Is Hair Transplant Appropriate?
Once traction alopecia has progressed to permanent hair loss — follicles destroyed and replaced by scar tissue — no medical treatment can restore hair. Hair transplant is then the appropriate intervention.
Ideal traction alopecia candidate for transplant:
- Causative hairstyle has been stopped
- Hair loss is stable (no active progression)
- The remaining scalp donor zone is not affected by traction
- Realistic expectations about the coverage achievable
Because traction alopecia typically affects the frontal hairline specifically, transplant results can be excellent — restoring a natural hairline in an area where patients are most bothered by the loss.
