Alcohol is one of the substances most commonly asked about in the pre- and post-operative period. The guidance is clear: alcohol should be avoided before surgery and during the critical healing phase. Here's why, and how long the restriction applies.
Why Alcohol Is Problematic Around a Hair Transplant
Blood thinning: Alcohol inhibits platelet aggregation — the process by which blood clots form. During and after surgery, normal clotting is essential to protect graft sites. Alcohol consumption in the days before or after the procedure can lead to prolonged bleeding, increased bruising, and slower wound closure.
Vasodilation: Alcohol causes blood vessels to dilate. While this sounds benign, it increases swelling and can elevate blood pressure at the graft sites, potentially displacing newly implanted follicles before they've anchored.
Immune suppression: Even moderate alcohol consumption temporarily suppresses immune function. During the healing period, your immune system is working to protect thousands of micro-wounds from infection. Any reduction in immune efficiency increases infection risk.
Dehydration: Alcohol is a diuretic. Dehydration reduces blood volume and impairs the delivery of oxygen and nutrients to healing tissue. Graft survival depends on adequate tissue perfusion.
Drug interactions: Post-operative medications — particularly antibiotics and anti-inflammatory drugs — interact adversely with alcohol. Mixing alcohol with these medications can cause side effects ranging from nausea to serious liver stress.
How Long Before Surgery Should You Stop?
Stop alcohol consumption at least 7 days before your procedure. This allows platelet function to normalise and reduces the risk of intraoperative bleeding complications.
If you are a regular or heavy drinker, inform your clinic during the pre-operative consultation. Your surgeon may advise a longer abstinence period.
How Long After Surgery Should You Wait?
The standard guidance is to avoid alcohol for a minimum of 10–14 days after your procedure. Many clinics recommend one month to be fully safe.
The rationale: during the first two weeks, grafts are in their most vulnerable phase — not yet anchored and entirely dependent on optimal tissue conditions for survival. Alcohol-related vasodilation, swelling, and immune suppression all pose risks during this window.
From week 3 onwards, with your surgeon's approval, moderate alcohol consumption can typically resume. "Moderate" means 1–2 drinks on an occasional basis — not a return to heavy or binge drinking during the first month.
What About a Celebratory Drink on the Night After Surgery?
This is a question that comes up more often than you might expect, particularly from patients who have flown to Istanbul with friends or family. The answer is a firm no. The first 24–48 hours are the highest-risk window for graft dislodgement and bleeding complications. Even a single drink can meaningfully elevate these risks.
Practical Advice for Social Situations
If you're attending social events during your recovery, alcohol-free alternatives are widely available. Many patients find it helpful to plan the timing of their procedure to avoid major social commitments in the first two weeks post-operation.
If you're travelling to Istanbul for your procedure, the city's excellent restaurants, cafes, and cultural sites offer plenty of enjoyable ways to spend recovery evenings without alcohol.
