The comparison between DHI (Direct Hair Implantation) and Sapphire FUE is one of the most common questions from patients researching hair transplantation. Both are FUE-based techniques — the extraction phase is identical. The difference lies in how grafts are implanted.
Standard FUE: The Baseline
In standard FUE, recipient sites (small incisions) are created first across the entire recipient area, then grafts are placed into those pre-made sites. This is the classic two-step implantation approach.
Sapphire FUE: What Is Different?
Sapphire FUE uses sapphire blades (made from synthetic corundum — the same material as sapphire gemstones) instead of steel blades to create recipient sites. The claimed advantages:
- Sharper, smoother cuts: Sapphire blades are harder and sharper than steel, allowing finer, smoother incisions
- Reduced tissue trauma: Smaller, cleaner cuts are proposed to reduce scalp trauma and post-operative crusting
- Potentially smaller site size: The blade geometry allows very fine slits
The actual clinical evidence for meaningful outcome differences between sapphire and steel blade FUE is limited. The technique is otherwise identical to standard FUE.
DHI: What Is Different?
DHI uses a specialised implantation device called a Choi pen (a hollow needle with a plunger). Grafts are loaded directly into the Choi pen and implanted simultaneously — there is no pre-made incision step. The pen creates the channel and implants the graft in a single motion.
Claimed DHI advantages:
- No pre-made channels: Reduces graft out-of-body time in the implantation phase
- No shaving required for some cases: Because no pre-made channels are created, DHI can be performed without shaving (useful for women or patients wanting to minimise visible change)
- Higher implantation density in small areas: The precise Choi pen allows dense placement in small areas
DHI is genuinely different in implantation mechanics. The technique requires a skilled team experienced with the Choi pen — learning curve is real.
