Follicular Unit Extraction (FUE) is the dominant hair transplant technique worldwide, having largely replaced the older FUT (strip) method. Despite its prevalence, many patients begin their consultation with only a vague understanding of what the procedure actually involves. This guide explains FUE step by step.

What Is FUE?

FUE is a surgical hair restoration technique in which individual follicular units — naturally occurring groups of 1–4 hair shafts — are extracted one by one from a donor area (typically the back and sides of the scalp) and implanted into areas of thinning or baldness.

Unlike FUT, which removes a strip of scalp skin, FUE leaves no linear scar. Each extraction site heals as a tiny circular dot, which becomes invisible once the surrounding hair grows back.

Step 1: Pre-Operative Assessment and Design

Before surgery begins, the surgeon assesses the donor area to determine density and quality, estimates graft requirements, and designs the recipient hairline and zone. The hairline is drawn on the scalp with the patient upright and awake.

Step 2: Shaving the Donor Area

For standard FUE, the donor area (back and sides of the scalp) is shaved to approximately 1mm length. This allows the surgeon to see each follicular unit clearly and extract grafts without the overlying hair obstructing access. Some clinics offer "unshaven FUE" for patients requiring maximum discretion, though this is more technically demanding.

Step 3: Local Anaesthetic

The scalp is numbed with a local anaesthetic solution injected around the donor and recipient zones. This is the most uncomfortable part of the procedure — most patients describe it as a series of sharp pinches. Once the anaesthetic has taken effect (usually within 10–15 minutes), the scalp is fully numb.

Step 4: Follicular Unit Extraction

Using a specialised micro-punch (typically 0.6–1.0mm in diameter), the surgeon scores around each selected follicular unit, creating a circular incision that frees the graft from the surrounding tissue. The graft is then extracted with fine forceps.

This is the most technically demanding phase. The punch must be angled precisely to match the hair's growth direction beneath the surface — an incorrectly angled punch transects the follicle and destroys it.

Grafts are collected into a chilled preservation solution as they are extracted, keeping them viable while awaiting implantation.

Step 5: Graft Sorting and Preparation

Once a sufficient number of grafts have been extracted, the team sorts them by follicular unit type — single-hair grafts for the hairline, multi-hair grafts for density zones. Any damaged grafts are identified and removed.

Step 6: Recipient Site Creation

Tiny incisions are made in the recipient area using fine blades or needles. The size, depth, angle, and direction of each incision determines the final appearance of the transplanted hair. In Sapphire FUE, these incisions are made with sapphire-tipped blades rather than steel, which are claimed to produce more precise, less traumatic channels.

Step 7: Graft Implantation

Prepared grafts are implanted into the recipient channels one by one. In standard FUE, this is done with fine forceps. In DHI (Direct Hair Implantation), a specialised pen device (Choi implanter) combines channel creation and implantation in a single step.

This phase requires patience and precision — each of the thousands of grafts must be placed at the correct depth, angle, and direction.

Step 8: Post-Operative Care

Once all grafts are implanted, the scalp is gently cleaned and bandaged if required. The patient rests briefly and receives written aftercare instructions before leaving the clinic.

How Long Does FUE Take?

A typical FUE session of 2,000–3,000 grafts takes 6–8 hours. Larger sessions (4,000+ grafts) may take 8–10 hours or be split across two days.
