Diabetes is one of the most common underlying health conditions among hair transplant patients. Well-managed diabetes does not necessarily preclude transplantation, but it requires specific assessment, preparation, and modifications to surgical management.
How Diabetes Affects Hair Transplant Outcomes
Impaired wound healing: Elevated blood glucose impairs all phases of wound healing — inflammation, proliferation, and remodelling. Diabetic patients typically heal more slowly and have higher rates of post-surgical complications including infection.
Reduced microvascular circulation: Diabetic microangiopathy — damage to small blood vessels — reduces blood flow to tissues. Transplanted grafts depend critically on receiving adequate blood supply from surrounding tissue. Impaired microcirculation reduces graft survival rates.
Increased infection risk: Diabetes impairs immune function, increasing susceptibility to surgical site infections. The thousands of micro-wounds created by FUE extraction and implantation each represent potential infection entry points.
Neuropathy: Diabetic neuropathy can affect scalp sensation and may complicate local anaesthetic administration and post-operative pain assessment.
Well-Controlled vs Poorly Controlled Diabetes
The key variable is control. Patients with well-controlled diabetes (HbA1c below 7.5%) generally tolerate hair transplantation safely with appropriate precautions. Patients with poorly controlled diabetes (HbA1c above 8.5–9%) face significantly elevated risks.
Most surgeons will not perform elective hair transplantation on patients with HbA1c above 8.5% until glycaemic control is optimised.
Pre-Operative Requirements for Diabetic Patients
Before proceeding, diabetic patients should:
- Provide recent HbA1c results (within 3 months)
- Confirm current medication regimen with their treating physician
- Ensure blood glucose is well-managed in the days leading up to the procedure
- Discuss medication management on the day of surgery with their physician (particularly insulin and oral hypoglycaemics)
- Have a pre-operative clearance from their treating physician if taking blood-thinning medications
On the Day of Surgery
Blood glucose should be monitored before, during, and after the procedure. The clinical team should be informed of diabetes status so appropriate protocols can be followed. Stress (including surgical stress) can cause glucose fluctuations — this must be anticipated and managed.
Post-Operative Care for Diabetic Patients
Post-operative care is generally similar but with heightened attention to:
- Signs of infection (increased redness, warmth, swelling, or discharge beyond the normal healing response)
- Adherence to antibiotic course
- Wound inspection at earlier follow-up intervals
- Blood glucose monitoring during the healing phase
Diabetic patients should complete their antibiotic course without exception and contact their clinic immediately if they have any concerns about healing.
Realistic Expectations
Diabetic patients who are well-controlled can achieve results comparable to non-diabetic patients. The risk of slightly higher graft loss and slightly slower healing should be acknowledged. Poorly controlled diabetes significantly increases these risks.
