When patients ask about hair transplant success rates, what they're really asking about is graft survival — the percentage of transplanted follicles that successfully establish themselves in the recipient area and produce permanent hair. Understanding what influences graft survival helps you evaluate clinics, understand your surgeon's technique, and take the right steps in your own recovery.
What Is a Good Graft Survival Rate?
In experienced hands with appropriate technique, graft survival rates of 90–95% are achievable. This means that out of 3,000 transplanted grafts, approximately 2,700–2,850 will survive and produce permanent hair.
Below 80% is generally considered a poor result. Clinics that achieve consistently high survival rates do so through a combination of surgical precision, correct graft handling, and appropriate patient selection.
Factors That Determine Graft Survival
1. Surgeon Skill and Experience
The extraction phase of FUE requires precise control to harvest follicles intact without transection (cutting through the follicle). Transected grafts cannot produce hair. An experienced surgeon extracts grafts cleanly and efficiently; an inexperienced one may transect 10–20% of extracted follicles.
The implantation phase requires correct depth, angle, and direction for each graft — thousands of individual decisions that determine whether the transplanted hair grows in a natural pattern.
2. Time Outside the Body (Ischemia Time)
Grafts removed from the scalp are living tissue. The longer they remain outside the body without adequate oxygenation and nutrients, the higher the cell death rate. Minimising the time between extraction and implantation — and keeping grafts in an appropriate holding solution at the correct temperature — is critical.
In well-run procedures, grafts are implanted within 4–6 hours of extraction. Clinics running very large sessions (5,000+ grafts in a single day) with inadequate staffing may exceed this window.
3. Holding Solution
Grafts stored in chilled saline survive longer than those stored at room temperature. More advanced clinics use specific preservation solutions (such as HypoThermosol or PRP) that have been shown to improve graft survival rates compared to plain saline.
4. Handling Technique
Grafts must be handled only by the fatty tissue at the base of the follicle — never by the hair shaft or upper follicle, which causes mechanical damage. In clinics where untrained technicians handle grafts, damage rates are higher.
5. Patient Aftercare
Once implanted, graft survival depends on the patient following aftercare instructions correctly. The main aftercare risks to graft survival are:
- Dislodgement: Direct contact, scratching, or high-pressure water on the recipient area in the first 10 days
- Infection: Poor hygiene or failure to complete the antibiotic course
- Smoking: Impairs circulation and graft oxygenation
- Alcohol: Increases swelling and impairs healing
- Early intense exercise: Raises blood pressure and can cause micro-bleeding at graft sites
6. Patient-Specific Factors
Some factors are beyond the surgeon's or patient's control:
- Scalp laxity: A tighter scalp has reduced blood supply, which can affect graft integration
- Skin type: Patients with very oily or very dry skin may face different healing challenges
- Underlying health conditions: Diabetes, autoimmune conditions, and poor nutritional status all impair wound healing
- Medications: Some medications (blood thinners, immunosuppressants) affect healing
How Is Graft Survival Assessed?
Graft survival cannot be directly counted after the procedure — there is no non-invasive method to determine exactly how many grafts have survived. Instead, results are assessed at 9–12 months by comparing the density and coverage achieved with the number of grafts transplanted.
An experienced surgeon can estimate survival quality from the growth pattern visible at the 6–12 month follow-up.
Red Flags for Poor Graft Survival
Be cautious of clinics that:
- Run sessions of 5,000+ grafts with minimal staff
- Use technician-only teams without qualified surgical oversight
- Cannot provide verifiable before/after results showing consistent density
