Diabetes is one of the most common chronic conditions among hair transplant patients. Understanding how diabetes affects the procedure — and what steps minimise risk — allows diabetic patients to make informed decisions.

Does Diabetes Cause Hair Loss?

Yes, diabetes is associated with hair loss through several mechanisms: reduced microvascular circulation to the scalp (impairing follicle nutrition), peripheral neuropathy effects, and the hormonal/metabolic dysregulation of poorly controlled diabetes. However, androgenetic alopecia in diabetic patients is treated the same way as in non-diabetic patients.

Can Diabetic Patients Have Hair Transplant?

Yes — with appropriate precautions. Diabetes alone is not a contraindication to hair transplantation. The key question is how well controlled the diabetes is.

Well-controlled diabetes (HbA1c < 7.5–8%): Patients with stable, well-controlled diabetes are generally suitable candidates. Risk is modestly elevated compared to non-diabetic patients but manageable.

Poorly controlled diabetes (HbA1c > 8–9%): Higher risk of impaired wound healing, infection, and reduced graft survival. Surgery should be delayed until glycaemic control is improved.

Specific Risks in Diabetic Patients

Wound healing: Elevated blood glucose impairs the wound healing cascade. Minor healing complications are more likely. Meticulous post-operative care is essential.

Infection risk: Elevated glucose creates a more favourable environment for bacterial growth. Prophylactic antibiotics are typically extended for diabetic patients.

Graft survival: Microvascular disease reduces blood supply to the scalp — potentially reducing graft survival rates compared to non-diabetic patients.

Hypoglycaemia risk: The stress of the procedure and fasting requirements can affect blood glucose levels. Glucose monitoring during the procedure is standard practice.

Pre-operative Requirements for Diabetic Patients

  • Recent HbA1c result (within 3 months)
  • Fasting and post-meal blood glucose readings
  • Optimised glycaemic control in the lead-up to surgery
  • Coordination with the patient's endocrinologist or GP
