How Female Hair Loss Differs from Male Hair Loss

The Most Common Causes
Female Pattern Hair Loss (FPHL): The most common cause, affecting women of all ages. Characterised by diffuse thinning across the central scalp with preservation of the frontal hairline. Influenced by genetics and hormones.
Telogen Effluvium: Temporary diffuse shedding triggered by systemic stress — childbirth, significant weight loss, major illness, nutritional deficiency, or hormonal changes. Typically begins 2–4 months after the trigger and resolves within 6–12 months.
Alopecia Areata: Autoimmune condition causing patchy, non-scarring hair loss. Can progress to total scalp loss in severe cases.
Traction Alopecia: Caused by tight hairstyles (braids, extensions, tight ponytails). Initially reversible; permanent if traction is sustained over years.
Hormonal Causes: Thyroid disorders, PCOS, and menopause can all cause or accelerate hair loss — often overlooked in diagnosis.
Nutritional Deficiencies: Low ferritin (iron stores), vitamin D deficiency, zinc deficiency, and inadequate protein intake all impair follicle function.
Diagnosis
A thorough workup includes medical history, physical examination, blood tests (ferritin, thyroid, vitamin D, hormonal profile), trichoscopy (dermoscopic scalp examination), and in unclear cases, scalp biopsy.
Treatment Options
Minoxidil: Most evidence-based treatment for FPHL. Topical 2% or 5% applied daily, or oral low-dose minoxidil — both reduce shedding and stimulate regrowth.
Treating underlying causes: Correcting iron deficiency, thyroid disorders, or nutritional deficiencies often produces significant improvement.
PRP: Platelet-rich plasma injections stimulate follicle activity and reduce shedding — used alone or alongside other treatments.
Hair Transplant: For women with stable pattern hair loss and adequate donor density, transplant is viable. The diffuse nature of female pattern loss requires careful patient selection.
