What Minoxidil Does (and Doesn't Do)
Minoxidil is a topical solution (or foam) applied directly to the scalp. Originally developed as a blood pressure medication, its hair growth side effect was discovered and it has been used for hair loss since the 1980s.
What it can do:
- Slow or pause hair loss in the early-to-mid stages of androgenetic alopecia
- Stimulate some regrowth in miniaturised follicles that haven't yet been lost
- Maintain existing hair density when used consistently
What it cannot do:
- Restore hair in areas where follicles have been permanently lost
- produce the same density as a hair transplant
- Work without continued use — stopping minoxidil causes the hair it's maintaining to shed within 3–6 months

What a Hair Transplant Does
A hair transplant permanently relocates healthy, DHT-resistant follicles from the donor area (typically the back and sides of the scalp) to areas of thinning or baldness. These relocated follicles continue producing hair for life.
What it can do:
- Permanently restore density in areas of loss
- Rebuild the hairline with natural-looking results
- Achieve this in a single session (with additional sessions possible as loss progresses)
What it cannot do:
- Stop ongoing hair loss in non-transplanted areas — this requires medication
- Work if the donor area is insufficient (very advanced loss)
- Produce results overnight — full results take 12–18 months
How to Decide: A Framework
You may be a good candidate for minoxidil alone if:
- Your hair loss is in the early stages (Norwood 1–2)
- You have active, healthy follicles that are miniaturising but not yet lost
- You are committed to daily, lifelong application
- You are not yet ready for surgery
You may be a good candidate for a hair transplant if:
- You have areas of established hair loss where follicles are gone
- Your hair loss has stabilised (or you're prepared to address future loss)
- You want a permanent solution rather than ongoing maintenance
- You are in good general health and a suitable surgical candidate
Most patients benefit from both: A hair transplant restores lost density permanently; minoxidil (combined with finasteride if appropriate) maintains native hair in surrounding areas and reduces the chance of needing further sessions.
The Role of Finasteride
Finasteride is a prescription oral medication that reduces DHT — the hormone responsible for androgenetic hair loss. Unlike minoxidil (which works locally), finasteride addresses the underlying hormonal cause. It is more effective than minoxidil at preventing further loss, but requires a prescription and has potential side effects that should be discussed with a physician.
Many hair transplant surgeons recommend finasteride post-transplant to protect remaining native hair and reduce the need for additional procedures.
