Hair transplantation for curly, wavy, or afro-textured hair requires modified technique compared to straight hair. The unique structure of these hair types — both in the follicle and the hair shaft — presents specific technical challenges that not all surgeons are equally equipped to handle.

Why Curly Hair Is Technically More Challenging

Follicle curvature: The follicles of curly and afro-textured hair follow a curved path beneath the skin surface, rather than a straight downward trajectory. During FUE extraction, the punch must follow the curved path of the follicle precisely — an incorrectly angled extraction transects (cuts through) the follicle and destroys it. Transection rates can be significantly higher in curly hair when performed by inexperienced surgeons.

Scalp characteristics: Patients of African and Afro-Caribbean descent often have scalp characteristics — including tendency toward keloid scarring — that require specific consideration in surgical planning and aftercare.

Visual assessment difficulty: The curl of the hair shaft above the skin does not directly indicate the direction of the follicle below. Accurate angle assessment requires experience and careful pre-operative mapping.

Technique Adaptations for Curly Hair

Experienced surgeons use several adaptations:

Smaller punch size: A smaller micro-punch (often 0.6–0.75mm rather than 0.8–1.0mm) allows more precise tracking of the curved follicle path.

Slower extraction pace: Rushing extraction of curly hair increases transection risk. Skilled surgeons pace extraction more carefully with curly and afro-textured hair.

DHI/Choi pen implantation: For afro-textured hair, DHI implantation (using a Choi pen) can be particularly advantageous because the implanter pen allows precise depth and angle control, which is important when transplanting into tight, dense hair.

Hydration: Keeping curly hair well-hydrated before and during the procedure helps with follicle visibility and extraction accuracy.

Results for Curly Hair

One natural advantage of curly hair in transplantation is the visual density benefit — each curly hair shaft covers more surface area than a straight hair, meaning curly-haired patients often achieve excellent visual density with fewer total grafts than straight-haired patients.

Results for curly hair transplantation, when performed by experienced surgeons, are excellent and natural-looking. The transplanted hairs retain their curl and texture.

Keloid Scarring Risk

Patients of African descent have a statistically higher tendency toward keloid scarring. This is relevant for donor area healing and should be discussed with the surgeon pre-operatively. A test extraction and monitoring for keloid response before a full session is sometimes advisable.
